IU HEALTH Athletic Physical Night at Cowan Jr/Sr High School - FREE

IU Health will be sponsoring an Athletic Physical Night at Cowan Jr/Sr High School on Wednesday, May 21, 2025 from 5pm-7pm. The cost is FREE for all 2025-26 5th graders through seniors. Both the IU Health consent form and the IHSAA Pre-Participation Physical Evaluation forms must be signed by the parent before students can be evaluated. It will be first come, first serve. Parents do not have to be present, but are welcome to come. All forms are available at cowanathletics.com and the main office of the elementary and high school.
